Join the practice
The medical centre will accept applications from people living in Abercynon and Mountain Ash. To register please call at the health centre with your medical card. If you do not have a medical card, the receptionist will give you a Form GMS1 to complete. You can also click here to download the GMS1 form please bring the completed form to the practice.
You will then be asked to make an appointment with the practice nurse for a general health check (you should bring a sample of urine with you to this appointment).
Those accepted onto the practice list will be eligible to receive the following standards of service:
- To make an appointment to see a doctor for treatment.
- To express a preference to see a particular doctor for any treatment.
- To make a request for an emergency appointment where necessary and you will be seen by whichever doctor is available on the day.
- To make a same day appointment for children who are sick and need to be seen on the same day.
You are entitled to consult a doctor anywhere in the UK if you are away from home and in need of medical help. You may do this by asking to see the doctor as a temporary resident and completing the necessary GMS3/99 form.
