Join the practice

The medical centre will accept applications from people living in Abercynon and Mountain Ash. To register please call at the health centre with your medical card. If you do not have a medical card, the receptionist will give you a Form GMS1 to complete. You can also download a GMS1 from the gov.uk website please bring the completed form to the practice.

You will then be asked to make an appointment with the practice nurse for a general health check (you should bring a sample of urine with you to this appointment).

Those accepted onto the practice list will be eligible to receive the following standards of service:

  • To make an appointment to see a doctor for treatment.
  • To express a preference to see a particular doctor for any treatment.
  • To make a request for an emergency appointment where necessary and you will be seen by whichever doctor is available on the day.
  • To make a same day appointment for children who are sick and need to be seen on the same day.
 

Temporary Residents

You are entitled to consult a doctor anywhere in the UK if you are away from home and in need of medical help. You may do this by asking to see the doctor as a temporary resident and completing the necessary form available on the NHS website

 

GP Registrars and Medical Students

We are a training practice, and you may be offered an appointment with a GP registrar. These doctors are fully qualified and will have had experience in several hospital specialties. They are completing their specialist training as a GP.
We also teach medical students and sometimes you may be asked to see the doctor or nurse with a student present.

You will always be asked beforehand if this is acceptable and you may ask for the student to withdraw if you prefer to have your consultation in private.
